If the above-listed Participant is under 18 years of age, I hereby declare that I am authorized as his or her guardian to sign this Release of Liability Form on his or her behalf, and understand and agree that they are bound by all terms and conditions of this document.
In consideration of the service provided by Drone Building Camps (*DBC), at a Drone Building Summer Program, I, as Participant or, if Participant is a minor, as parent/guardian of Participant agree as follows:
I am familiar with the Program offered by *DBC and all of my questions about the Program, including questions concerning the details of activities, have been answered to my satisfaction. I understand that participation in the *DBC’s Program creates a risk of injury and I expressly acknowledge and assume the risk of such injury to the Participant.
I represent that the Participant is fully capable of participating in this Program. Therefore, I assume and accept full responsibility and assume the risk of and for any injury, death, loss of personal property (including Participant’s Drone and all/any other parts/components provided to participant by *DBC), and/or expenses that may result from Participant's involvement in this *DBC Program, and I further agree to indemnify and hold harmless the Released Parties, Drone Building Camps and all entities associated with it to the fullest extent of the law, from any and all claims, damages, losses, or liability that may result from Participant's involvement in the *DBC Program.
Additionally, I affirm that the Participant must be focused and attentive at all times, and should not disturb any *DBC Program. Instructors reserve the right to permanently remove any Participant from a camp, with no right to reimbursement. In correspondence to these policies, a parent/guardian figure should be available to be contacted within 15 minutes of any incident, and must pick up the Participant within 45 minutes of being contacted. In the case that the Participant meets or exceeds the age of 18 years of age, and if a parent figure is not available for a pickup, he/she must leave the camp within no more than 30 minutes of the occurrence of the first conflict.

Then please make a copy, or print out both waivers, complete them, and either email them to info@dronebuildingcamps.com or bring them in on the day of the camp. Participants will not be allowed to join the programs without a waiver brought in at the beginning of the camp. Blank waivers will also be available at the beginning of the camp.

I, (Name of Parent/Guardian)___________________________, give Drone Building Camps (*DBC), permission to use my child’s name and image, as such may be embodied in any pictures, photos, video recordings, digital images, and the like, taken or made on behalf of *DBC or other Drone Building activities. I agree that *DBC have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with the *DBC’s missions. These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional or educational materials in any medium now known or later developed, including the Internet. I acknowledge that I will not receive any compensation, etc. for the use of such pictures, etc., and hereby release *DBC and its assigns from any and all claims which arise out of or are in any way connected with such use.
I have read and understood this consent and release.
I give my consent to the *DBC to use my child’s image and likeness to promote the DBC program, and/or its activities.
